Three parts, in plain language

Privacy, cookies and complaints.

How we look after your records, what this website stores in your browser, and what to do if something has gone wrong. Questions about any of it go to [email protected].

1. Privacy notice

1.1 Who is responsible

Quarry Hill Dental, 14 Quarry Hill, Leeds LS2 7EY, is the controller of the personal data described here.

1.2 What we hold

1.3 Why we use it

To provide your dental care, to book and remind you of appointments, to take payment, and to meet our legal and professional duties. Health information is used only for your care and is not shared with anyone outside the practice without your consent, unless the law requires it or you are referred to another clinician.

1.4 How long we keep it

Clinical records are kept for as long as professional guidance for dental records requires. Enquiries sent through the booking form that do not lead to an appointment are deleted once they are dealt with.

1.5 Your rights

You can ask for a copy of your records, ask us to correct them, or object to how they are used. Write to [email protected] or hand a letter to reception. If you are unhappy with our answer, you can complain to the Information Commissioner's Office.

3. Complaints

If you are unhappy with your care or how you were treated, tell us. Complaints are handled by Dr Amara Osei, the principal dentist, and making one never affects your treatment here.

  3. Within 20 working days

You receive a written reply with what we found and what we will do. If we need longer, we tell you why and when to expect it.
